Medicare reimburses for Skilled Level Care only.
Intermediate Care Facility - Provides health related services that do not require
hospital/skilled care nursing.
Assisted Living Facility - provides housing and personal services to those unable to live
independently.
Resident Care Facility - pts. unable to live independently. AKA Independent Living
Facility/Retirement Center.
OTHER CARE SETTINGS
Hospice Care - support to terminally ill pt and families.
Home Health Care - provides health, social and other supporting services to individuals
in their home. Must be ordered by physician.
Ambulatory care - Health care services provided in an outpt setting. Ex. PCP,
freestanding surery centers, walk-in clinics.
Hospital Outpt - Pt comes to the hospital for svcs but is not admitted as inpt.
Hospital Ambulatory - Clinics, ambulatory surgery, lab, radiology, therapy and
pharmacy.
OUTPT CLASSIFICATIONS - 3 Categories
Emergency Otpt. - Treatment of a condition that requires immediate service (Life/Death
situations)
Clinic Outpts. - When a clinic holds regularly scheduled clinics.
Referred Outpts. - pt comes to receive exclusive services (diagnostic or therapeutic
services from a hospital).

Principal Diagnosis
The condition established AFTER study which is the reason why the patient is admitted
to the hospital.
Principal Procedure

necessary for treating a certain condition. It is usually related to the primary diagnosis.
Complications
a condition that developed after hospital observation/admission that requires additional
eval and / treatment and prolongs the pts stay by at least one day.
Comorbidity
a "pre-existing condition that will increase the pts length of stay by at least one day in
75% of cases."
Provisional/Admitting Diagnosis
the diagnosis recorded at the time of admission. A very tentative diagnosis before H&P
is completed. Can also be phoned in prior to the pts arrival by the pts physician.
Impression
the diagnosis recorded at the end of the H&P, but before any tests/studies.
Differential Diagnosis
At conclusion of H&P, several diagnoses in mind.
