, The case for the IPO
Valery Van Houten (60) was diagnosed with RA two years ago. Initially she noticed the swelling
of her MCP and PIP joints in both of her hands. Pain was also present in those areas. Only when
she felt that her hands were getting stiffer, did she realize that something must be wrong. She
went to the GP, who referred her to the rheumatologist. There she received an official
diagnosis. Mrs. Van Houten is currently taking Methotrexate (anti-rheumatic drug). Since her
diagnosis she goes less and less out of her apartment. It seems like everything is painful for her,
even household chores are exhausting (PSC 7).
She developed a so-called “Swan-neck deformity”. It is not out of the ordinary that Mrs. Van
Houten has to take a nap twice a day, since she feels tired. She used to go for long walks with
her friend, but she had to stop because she now also has pain and swelling in the joints of her
feet. Therefore, she went to her rheumatologist again, who did an X-Ray of the affected joints.
Severe cartilage damage was visible in her hands and slight deformities in her feet. This made
her unmotivated and depressed.
Due to this, instead of a pack of cigarettes per week, she smokes one per day. She doesn’t think
that this is a problem since smoking decreases her appetite. She lost five kilos since the X-Ray
which was 4 weeks ago. Mrs. Van Houten lives alone in a big house with a garden. Her husband
recently died from cancer, and she doesn’t have any children. She retired right before she got
diagnosed with RA – she was a real estate agent. Mrs. Van Houten wants to be pain-free again.
She can’t stand the limitations in her daily life.
Her request for help is to do her chores without any problems again.
Inspection: obvious swelling in joints in hands
Palpation: painful, no temperature differences, also swelling in feet
AROM/PROM: decreased in all affected joints during flexion.
Gait pattern: normal
