Initial applications - Answer- Must be signed and complete. If they are not complete they will be
returned to the applicant within 5 working days of the receipt
If applicants are married, who must sign the Application for Foster Family Home License? - Answer- both
spouses
CFS 597 A - Answer- Application for Foster Family Home license for non relatives
CFS 597R - Answer- Application for Foster Family Home License for relatives
When should the licensing representative submit the CFS 597A and the CFS 718? - Answer- Within 10
working days of the receipt from the foster family home license applicant
CFS 718 - Answer- Authorization for Background Check
ILS - Answer- Individual Licensing Summary
What should the initial licensing study include? - Answer- 1. Announced in person home site interview
with the applicant
2. Walk through of the home by the licensing representative to assure compliance with Rule 402
3. Any non compliance letter(s) citing a Child Care Act violation
4. Completion of the pre-licensure foster or adopt PRIDE and other approved training
5. Narrative Family Assessment (Home Study)
6. Completion of CANTS 22B, CFS 452 A, CFS 452, CFS 452-1, CFS 452-3, CFS 452-4, CFS 506-G, CFS 590
CANTS 22 B - Answer- Acknowledgement of Mandated Reporter Status
