Assessment is collection by the RN of comprehensive data pertinent to the patient’s
health or situation.

Elements of the nursing scope and practice:
1. collects data in a systematic manner, prioritizes data collection; involves the
patient and family
2. uses evidence-based assessment techniques
3. synthesizes data and documents findings
4. derives diagnoses based on assessment data
