Atrial Fibrillation
Most common arrhythmia, pt on anticoagulant
(most likely warfarin, check PT/INR 2-3.5)
Rhythm: Irregular
Rate: less than 100 (controlled afib) OR 100+ (afib
RVR)
P Wave: NONE/ABSENT, fibrillatory waves
PR Interval: not measurable
QRS Complex: Normal
Atrial flutter
2 Types: Stable and Unstable
Stable: Controlling the rate & rhythm.
Continue to monitor patient for signs &
symptoms of poor cardiac output.
Unstable: Synchronized cardioversion
Rhythm: atrial rhythm is regular. Ventricular rhythm can be regular or irregular.
Rate: Atrial 250-350 bpm | Ventricular rate depends on the ratio of impulses conducted.
P Wave: Saw-Tooth Appearance
PR Interval: Not measurable
QRS Complex: Normal (less than 0.12 seconds/3 small boxes or less)
Sinus rhythms (NSR, SB,
ST) Rhythm: Normal
Sinus/regular Rate: 60-100 bpm
P Wave: Present, regular. One P Wave for
each QRS. Upright, smooth, rounded.
PR Interval: Consistent. Btn 0.12-0.2 seconds
(3-5 small boxes)
QRS Complex: 3 small boxes of less
Sinus Bradycardia (SA node slower < 60bpm)
Rhythm: Regular
Rate: less than 60 pbm
P Wave: Present, regular. One P wave for each
QRS. Upright, smooth, rounded.
PR Interval: Consistent, Btn 0.12-0.2 seconds (3-5
small boxes)
QRS Complex: Less than 0.12 seconds (3 small boxes or less)
