Subjective: CC (chief complaint): “You are with them” HPI: PW is 42-year-old who was brought to the emergency department for evaluation under police order for locking herself in a closet and screaming loudly for over an hour. She had to be sedated by paramedics with Ativan for aggressive and violent behavior. She is uncooperative with treatments, tests and suspicious of everyone. She accuses the interviewer of spying on her, “having eyes and ears planted everywhere.” She stated that nowhere is safe. Past Psychiatric History: PW has been to the emergency room three times in the last two weeks and was admitted to the inpatient psychiatric unit sometime last year. • General Statement: We know that PW lost her parents within the last three years and her friend reports that it affected PW a whole lot. It appears that PW has been having mental breakdown same time her parents passed away, every year, for the last three years. • Caregivers (if applicable): She lives with her husband who is a truck driver and is gone most of the time. She has a sister who is five years older than her, but not much was said about their relationship or where she lives. She has a best friend, who checks on her regularly and her name is Felicia. • Hospitalizations: PW was admitted to the inpatient psychiatric unit sometime last year. • Medication trials: None noted, but she was given Ativan for sedation, and she is allergic to Clozaril • Psychotherapy or Previous Psychiatric Diagnosis: Not disclosed
