Subjective: CC (chief complaint): “I did ask for oxycontin” HPI: DP is a 47-year-old white female, who was sent in for evaluation because her primary physician is concerned about her use of drugs, which was prompted by her latest request for prescription oxycontin for elbow pain. Her primary physician is concerned that oxycodone may interact with the other drugs she takes. Past Psychiatric History: None reported. Substance Current Use and History: Tylenol #3, morphine, hydromorphone, Demerol, alcohol, marijuana, cocaine, ecstasy, LSD, tobacco, Xanax, Adderall, caffeine. Family Psychiatric/Substance Use History: Denied family mental health or substance use issues. Psychosocial History: DP moved to Everett, Washington from Russia with her parents when she was 16 years old. Currently lives in Boise, Idaho. She has younger 1 brother, 3 older sisters. Denied family mental health or substance use issues. No history of inpatient detox or rehab denied self-harm hx; Menses regular. uses condoms for birth control. Has fibromyalgia. She works part time cashier at Save A Lot Grocery Store. Dropped out of high school in 10th grade. Sleeps 5-6 hours on average, appetite good. Medical History: Headaches, Fibromyalgia
