NRNP 6635 Week 10: Neurocognitive and Neurodevelopmental Disorders WINTER 2024 WALDEN UNIVERSITY
Subjective: CC (chief complaint): “I just can’t concentrate” HPI: HB is a 60-year-old Caucasian male, who was referred to the clinic for evaluation by his employer. He informed his supervisor that he was having problems concentrating and the supervisor then arranged for the consultation with the psychiatrist. HB stated that his problems are as a direct result of accelerated deadlines imposed by his employer, even though he admitted that the deadlines are affecting only him and not his colleagues. His main issue has been having difficulty focusing and concentrating, finishing projects, and organizing things. He even forgets to pay the bills on time. Past Psychiatric History: He has no psychiatry history even though he admitted that he had similar problems in college and as a child, but he was never diagnosed nor treated. • General Statement: HB has been very distracted, unable to focus, disorganized and all the above symptoms are affecting his productivity at work and at home. There is a lot of pertinent information that was not disclosed in this case and further assessment, interviewing is needed. • Caregivers (if applicable): He lives alone, single and dates casually. • Hospitalizations: None reported • Medication trials: None reported • Psychotherapy or Previous Psychiatric Diagnosis: None reported Substance Current Use and History: He drinks occasionally, one scotch drink on the weekends with a cigar. He denied history of drug use. Family Psychiatric/Substance Use History: None reported Psychosocial History: HB has a bachelor’s degree in engineering. He dates casually, never been married, no children. Has one younger brother. Sleeps 7 hours, appetite good. Denied legal issues Medical History: HTN, BPH, Angina, Hypertriglyceridemia Current Medications: Cozaar 100mg daily, ASA 81mg po daily, Valsartan 80mg daily, Fenofibrate 160mg daily, Tamsulosin 0.4mg po bedtime. Allergies: Dilaudid Reproductive Hx: No children ROS:
