controlled substances? - -Yes. A separate registration is required for
each principal place of business or professional practice at one general
location where controlled substances are manufactured, distributed,
used in research or scientific investigation, prescribed or dispensed.

numbered years

*C-2 in only these extreme cases: drug route is parenteral, LTC patient,
or hospice patient
-what happens if the physician address/DEA# is not on the
prescription? - -you can add it if you have the address on file/DEA# or
contact the doctor office. also add "RA" or "readily available" then initial
