Information obtained includes the patient's demographics, a hereditary condition, narrative discussion to understand the family pedigree, review of patient and family history, synopsis of major health and genetic disorders, and an educational summary on only one of the risk factors identified. Narrative Demographic identifiers The patient is a 58-year-old Asian male, of Korean descent. This history is obtained from the patient himself who is considered to be a reliable historian. He is currently employed as a technician in a dental laboratory company. He has been married for 30 years and has one daughter, ages 29. He lives with his wife now. The patient denies any current medical conditions or concerns. However, the patient came to see a primary care provider (PCP) because he had a simple physical check-up a few weeks ago at the Korean community center and found his blood pressure was elevated and high cholesterol (LDL). The blood pressure was 155/96 mmHg and total cholesterol was 260 mg/dL. This patent was born in South Korea. He graduated Theological Seminary in S. Korea and ministered as a church pastor for 10 years. In 2010, he moved to the US, and now is working in a dental laboratory office as a technician. He is a non-smoker and denied any recreational drug use. However, he drinks two to three cups of coffee a day and also drinks one glass of red wine once a week. The patient eats more fish, chicken, vegetables, and fruit than beef and lamb. However, the patient likes to eat Korean style salted fish and preserved food like kimchi. He likes jogging and exercises two to three times a week, including cardio and strength training. The patient attends Korean Christian services weekly with family. The patient has a past medical history of right knee replacement and appendectomy surgery. 3 GENEALOGY DISCUSSION The patient denied any recent headache, pain, chills, fatigue, night sweats, difficulty in breathing, or vision change. He also denies hearing loss, ear pain, or discharge from the ear, and tinnitus. The patient denies any changes in smell but does experience seasonal allergies, therefore has sinus pain occasionally. The patient denies any loss of consciousness and dizziness. No coughing, wheezing, cyanosis, dyspnea, or hemoptysis is reported by the patient. The patient also denies seizure, numbness, or tremors, falls. He expresses some anxiety about the diagnosis and treatment of hypertension and hyperlipidemia. Otherwise the patient's overall health condition is good. He does not currently take any prescribed medication, but he takes vitamin C, 2000 mg three times a day. He also has an updated immunization, including BCG for TB, hepatitis A & B vaccines, and yearly flu vaccine. The patient denies physical, mental, and verbal abuse. He has never received blood transfusions and had routine annual labs (CBC, CMP, Lipid Panel, Vit D, TSH, UA, PSA). The primary health disorders identified are hyperlipidemia and hypertension. Family History His parents are still alive and married for 62 years. His father is 86 years old and his mother 81 years old. He has a medical history of stomach cancer surgery 15 years ago, and hypertension and hyperlipidemia, diagnosed in 1995. He has been taking Lisinopril and Atorvastatin, doses unknown. His mother is 81years old and has a history of hypertension with multiple hospitalizations for uncontrolled high blood pressure and severe headache. Also, she has been suffered from type 2 diabetes mellitus (DM2) and taking medications for 20 years. The patient has one sister who is still living. She is 55 years old and also has a DM2, which requires oral medication therapy. She has two children, a son and a daughter, ages 33 and 29 respectively. Neither has been diagnosed with DM2, however, his son is currently on statins due to hyperlipidemia and hypertension. No other medical conditions noted. All patient's grandparents from both sides have passed away a long time ago and the patient is not clear about their medical conditions.
