Notes:
(i) Read the associated signs and diagnose the case.
(ii) Answer the questions after carefully reading the text.
A patient with complaints of wheezing, cough, dyspnea, and short respiration has
been admitted to a clinic in the beginning of Spring season. The condition is
likely to be bronchial asthma, which is the most appropriate drugs to deal with the
condition?
1. Inhalation of a rapid-acting beta-2 adrenoceptor agonist, such as clenbuterol,
usually provides immediate bronchodilation.
2. Intravenous corticosteroids, such as methylprednisolone is also required to
control allergic reaction caused by pollen grains at the beginning of Spring
season and to inhibit further histamine release. Inhaled corticosteroids such as
beclomethasone treat chronic airway inflammation but do not provide immediate
effect.
3. Carbogen artificial respiration that is oxygen with 5% CO2 tension may be given.
Which medicament inhibits the action of 5-lipoxygenase (LOX) and consequently the
action of leukotrienes on the bronchial tree.
A) Cromolyn
B) Zafirlukast
C) Montelukast
D) Zileuton
The answer is (D).
Zileuton is the only 5-lipoxygenase inhibitor given. While zafirlukast and
montelukast both Inhibit the effects of leukotrienes, they do so by blocking the
LTs receptors. Cromolyn is a mast cell stabilizer, Inhibits mast cell
degranulation and the further release of histamine.
