A patient who has been suffering from heart disease was admitted to the clinic with
complaint of difficulty breathing that is diagnosed as acute pulmonary edema. Which
treatment is appropriate among those listed below?
A) Acetazolamide
B) Torsemide
C) Chlorothiazide
D) Amiloride
The answer is (B).
This case is serious and is life-threatening. The appropriate approach is to
administer a high efficacy diuretic. This will reduce fluid accumulation inside
the lungs and consequently improves oxygenation and heart function.
The loop diuretics such as Torsemide and Furosemide are most effective in
scavenging large fluid volumes from the body and, therefore, are the appropriate
treatment in this situation. It is advised to administer the drug parenterally.
Although other choices produce diuresis, but they are inappropriate because of
their low and moderate efficacies.
A patient with hepatic cirrhosis developed ascites (intraperitoneal edema), that is
resistant to some diuretics. Which diuretic is hoped to improve the condition?
Spironolactone is the drug of choice for the treatment of hepatic edema. The
condition is usually resistant to loop or thiazide diuretics, therefore, a
combination with spironolactone will be effective and beneficial.
