The answer is (A).
Eplerenone is a Potassium-sparing diuretic. It acts on the collecting tubules and
ducts via aldosterone antagonism to inhibit Na/K exchange (Na+ reabsorption and
K+ excretion). Patients on any potassium-sparing diuretic should be closely
monitored for their Potassium blood levels. Exogenous supplementary sources of
Potassium must be stopped during potassium-sparing diuretic therapy. The other
mentioned drugs, Acetazolamide, Chlorothalidone and Ethacrynic acid promote the
excretion of potassium in the urine with no fear from hyperkalaemia.
A male patient developed a complaint of enlarged breasts. Which among the following
diuretic medicaments is the most likely to be the cause?
A) Furosemide
B) Spironolactone
C) Triamterene
D) Hydrochlorothiazide
The answer is (B).
Spironolactone has the adverse drug action of gynecomastia due to its
antagonistic effects on androgen and progesterone receptors. Eplerenone may be a
suitable alternative if the patient is still in need of an aldosterone
antagonist.
