The Health History and Physical Examination: Health Assessment RUA A comprehensive health assessment is needed prior to admitting a patient in any healthcare facility. This consists of gathering a health history which is the process of obtaining subjective and objective medical information from the patient and their family. The purpose of doing this prior to any treatment for the patient is to assist in minimizing potential health concerns, address the problem at hand, and create an effective and safe treatment for the patient. Therefore, the purpose of this paper is to discuss a health history and physical examination of an individual. Subjective Data Demographic Data The patient that I choose to interview is a 68-year-old, Caucasian, female whose name will be referred to as D.Y. throughout this paper. The client was born in Chicago, Illinois but now lives in Westmont by herself. D.Y. is currently divorced and has three grown children who all live on their own. She has completed all years of schooling up to a bachelor’s degree in college and now has an occupation as a Clinical Application Specialist Senior. Since the patient’s primary language is English and speaks fluently with consistent answers, there was no need for a secondary source, and she was reliable. Perception of Health and Past Medical History Upon asking the patient to describe her general health, she stated, “I am relatively healthy for a person my age. I take vitamins and herbal sup
