"My patient is a young healthy-appearing Man. He is A&O - X4, Relaxed,
Sitting upright position, Well developed well-nourished and well-groomed with no
signs of distress and denies having any pain."
5. Head, Neck, & Skin Assessment
head:
1. Assess head is normocephalic (feel head with fingers)
2. Note any bumps, bruises, lesions, or scars (look)
3. Observe for distended neck veins (look at JVD)
Face muscles:
1. Assess for symmetry with facial muscles (look)
2. Check trachea is mid-line (Place your index finger on the trachea in
the sternal notch, then, Slip it off to each side Note for symmetric on
both sides.
f. Check Skin temp w/ backs of hands, color, turgor and cap refill, &
edema.
"The patient's skin is warm, dry, and intact with good turgor it is appropriate for ethnicity
& consistent in color. No pitted edema was noted in the lower extremities.
The head is round, normocephalic, and symmetrical, his
skull has no nodules, masses, or depressions when palpated. His face
appears smooth and has uniform consistency with no presence of
