The purposes of the arrangement conference are to:
1. Neutralize the family's feelings and build rapport
2. Obtain Vital Statistics
3. Plan the funeral
4 Sell merchandise

Burial Allowance (Form #21-530)
Must be filed withing 2 years of the date of final disposition
Bereaved family would receive $300 upon approval
Additional $150 if buried in a state owned cemetery

When should you apply for a flag?
Before the funeral takes place
Where can you obtain a flag?
At any first, second, or third class post office, at the fourth class post offices in the
county seats or at any field station of the Veterans Administration (VA),
including contact offices.
The order of preference for signing the VA Form # 9
0-2008 is:
1. Next of kin or other relative;
2. Person making the funeral arrangements;
3. Funeral director
4. Representative of a veterans' organization having charge of the funeral;
5. Other person having knowledge of the eligibility
of the deceased and acting in the interest of his
(or her) family.
The order of preference to entitlement of the flag:
,1. Widow or widower
2. Children, according to age
3. Parents, adoptive parents, stepparents and foster parents
4. Brothers or sisters, including half-blood siblings
5. Uncles or aunts;
6. Nephews or nieces;
7. Others, cousins, grandparents, etc.
If two relatives having equal entitlement request the flag?
It would go to the elder of the two
Can a flag be issued to a widow/widower who is separated from the deceased?
Yes. As long as the widow/widower is not remarried.
Can a flag be issued to a widow/widower who was separated from the deceased
and then remarried?
No. Unless the application for the flag was made prior to the remarriage.

What is the time limit for filing for a Headstone/Marker?
There is no time limit.
The nearest relative, funeral director or other person may request a headstone or
marker when:
When interment is in a cemetery other than a Veterans Administration or National
Cemetery

Statement of Death by Funeral Director
What is the purpose of Form #SSA 721?
1. Notifies Social Security that someone has died.
2. Starts the process for a family member or members to receive possible Social
Security benefits
Lump Sum Death Benefit:
1. The amount is $255.00
2. Paid to surviving spouse or dependent child only
3. Must be claimed within two years from the date of death
4. Social Security contacts the family via the Statement of Death From to file for the
LSDB, if eligible.
5. The Claimant's Form is SSA 8.
If both spouses are receiving Social Security benefits and one dies, will the
survivor receive both payment?
The survivor will receive whichever payment is larger, but will NOT continue to receive
both payments.
For direct deposit of a social security check, the bank will:
Have to be notified of the persons death.
