DEPRESSION IN CHILDREN

- persistent, impairing sadness, Anhedonia or irritability that is rela vely unresponsive to pleasurable
ac vi es and interac ons and a en on from other people.
DSM-5
Category of depressive disorders - DMDD, MDD, PDD, Premenstrual dysphoric disorder, subs/medi induced
depressive disorder, dep d/o due to another med condn, other dep d/o, unspeci ed dep d/o.
- dep d/o - never manic episode or hypomanic
At least 2 weeks of persistent change in mood, nearly every day, manifested by either depressed or irritable
mood,
or loss of interest or pleasure
AND
At least 4 addi onal symptoms
1. changes in appe te or weight or failure to make expected wt gain
2. sleep
3. psychomotor ac vity
4. Decreased energy
5. feelings of worthlessness or guilt
6. di culty thinking or concentra ng or indecisiveness
7. recurrent thoughts of death or suicidal idea on, plans, or a empts
With impairment of fn or clin sig distress.
Speci ers
Severity - mild, moderate, severe, with psycho c features
Course - in par al or full remission
Other speci ers
1. With anxious distress
2. With mixed features
3. With melancholic features
4. With atypical features
5. With mood-congruent psycho c features
6. With Catatonia
7. With peripartum onset
8. With seasonal pa ern
ICD-10
DSM-5- for children and adolescents- irritable mood and failure to make expected wt gain ICD-10- no such
speci c criteria for child
Also PDF in DSM5 - dura on for child is 1 year and for adult is 2y
But ICD-10- 2 y for all
Chronic depression - > 12 months
Brief recurrent depression - recurrent symptoms of dura on shorter than 2 weeks.
PDD - in children and adolescents is a chronically depressed or irritable mood for at least 1 year
DMDD - a new Dx in DSM-5
severe persistent irritability with temper outbursts occurring at least 3 mes weekly
in child who is at least 6y
Onset prior to 10y
Shud not be Dxd a er 18 years
