Performs patient related activities, using physical agents and in good standing under the direction of a
physical therapist, but shall not require onsite supervision of a PT

physiatrist?

Document of authorization granted by the board of PT & issue by the department of health
How often does a PT need to renew their license?
Every 2 years
PT license reactivation involves the following 3 criteria
1. Paid fees ($50) or others that may be required
2. Met CE requirements - not to exceed 10 hours per year of inactive license
3. Proof of practice for 4 years preceding in another jurisdiction OR pass exam
