Case scenario- peripheral vascular-: Ruth Livingston History of Present Illness Ms. Livingston is an 80-year-old female who was admitted to the orthopedic surgical unit from the emergency department because of a trip and fall accident at home. X-rays revealed a right femoral head neck fracture. Ms. Livingston has a history of osteoporosis. She went for an open reduction internal fixation (ORIF) late yesterday evening. Vital signs have been stable- 98.6 oral, 72 pulse, 20 resp, 136/74. Her pain is being controlled by IV morphine PRN. She was last medicated with 2 mg of morphine sulfate IV 30 minutes ago. Ms. Livingston also has methicillin-resistant Staphylococcus aureus in her urine. Currently having pain to right leg only during dorsiflexion. Nurse will assess for deep vein thrombosis. History and Physical Information Biographic Data: Ruth Livingston, Female, DOB: 10/8/1940 Allergies: Penicillin (PCN), reacts with itching and hives Prior Medical History: History of osteoporosis and is taking calcium and Fosamax. Recent Medical History: Ms. Livingston presented to the emergency department (ED) approximately 7 hours ago by ambulance from her home. X-ray reveal a right femoral neck fracture as a result of tripping and falling in her home. ORIF to R hip today 0800 Family History: Father deceased- History of HTN, COPD. Mother deceased- history of stroke Social History: Patient is married and lives with her husband in an older two-story house. Both are retired. She enjoys quilting and crocheting
