,Inleiding
Dit jaar loop ik mijn stage op het expertisecentrum van HilverZorg te Nieuw
Kerkelanden in Hilversum. Op deze afdeling worden bewoners met Korsakov
gehuisvest en voorzien van de juiste zorg die is afgestemd op de persoonlijke
wensen en behoeftes van elk individu.

Naast Korsakov komen ook psychische problemen voor en niet aangeboren
hersenletsel. Ook hebben wij bewoners met een rechtelijke machtiging die door een
rechter verplicht bij ons geplaatst zijn.

In dit portfolio voor mijn praktijkleren 2 kunt u de opdracht klinisch redeneren en
onderzoekend vermogen vinden. Voor mijn casus heb ik een zorgvrager gekozen
waarmee ik de meeste affiniteit had en het interessant vond om verder onderzoek
naar te doen. In mijn verslag noem ik de bewoonster mevrouw X.

Naast de opdrachten klinisch redeneren en onderzoekend vermogen zijn ook de
opdrachten van reflectie en ethiek bijgevoegd. Deze opdrachten hebben een aparte

Casusbeschrijving
Het betreft een 75-jarige vrouw met schizo-affectieve stoornis van het bipolaire type.
Eerder zijn een bipolaire stoornis en borderlinepersoonlijkheidsstoornis als diagnose
overwogen, later werd de diagnose bijgesteld naar schizofrenie van het paranoïde
type (eerste psychotische klachten op haar 21ste). Er is sprake van
persoonlijkheidsproblematiek met borderline en afhankelijke trekken, waarbij de
