• Rationale for the questions you asked during the history examination When it came to asking questions in the health history examination with Carolyn Cross, I used the format that we have been taught of OLDCARTS. Understanding this format and how to use it has aided me tremendously when it comes to health history examinations. Looking back at the feedback on health history I can see that I still need practice with limiting how many questions I ask the patient. It took me over 100 questions to get 26 out of the 29 expert questions to be asked. I find myself being able to get many of the questions based on the chief complaint and why the patient is coming to the clinic, but when I get down to having a few left I begin to start asking more questions that are not appropriate. This is a challenge for me and something that I need to work on but will come in due time. I asked the questions I asked based on why the patient was coming to the clinic. Since the patient was a former patient of the clinic, a total health history was not warranted, and the questions asked revolved around the notion of the patient being concerned about breast cancer. Following the format of OLDCARTS sent me down the pathway and allowed me to ask the appropriate questions required to illicit the correct responses. • Rationale for the physical exam that was conducted on the patient I started off with vital signs because every patient that presents to the clinic should have their vital signs taken and that includes height with weight. Depending on what the patient presents with, the vital signs might give us a clue about what might be going on or present another topic of discussion within the exam.
