leading cause of death among women in all ethnic groups worldwide. In the United
States, women have a 12.4% lifetime risk of being diagnosed with the condition
(American Cancer Society, 2017). One of the most afflicted ethnic groups is non-
Hispanic black women. Breast cancer incidence and death rates before the age of 40
are higher for non-Hispanic black women more than any other ethnic group (American
Cancer Society, 2017). Therefore, building and maintaining a comprehensive breast
cancer database is essential since this centralized repository for data helps clinicians
and researchers understand more about the etiology, diagnosis, and treatment for
cancer. When building the database, it is important to find and organize the
information required. This can be accomplished by utilizing the existing data points
found in the electronic health record (EHR) or by developing a form to capture the
