SC2 PAST YEARS

HOPI : Patient around his 50s with underlying hypertension and
hyperlipidemia presented because he collapsed a few hours ago
while he was in the supermarket bending to get something. Prior to
collapse, he felt chest tightness and shortness of breath. There was
no headache, blurring of vision, tinnitus, vertigo before the collapse.
He took all his meals for the day. No jerky movements, eye rolling
or tongue biting when he collapsed. Loss of consciousness for a
few minutes and he was brought to the ED. He had multiple angina
episodes prior to this.
DX : AORTIC STENOSIS (DIFF DX : MI, HYPOGLYCEMIA)
Questions :
1) What will you do when patient is in clinic ? -
Cardiovascular PE
2) What do you expect to find upon PE ?
- Murmurs, type of murmur specifically in AS
(describe the pulses expected)
3) Ix - ecg, lab, imaging
4) Mx - Surgical Options (valve replacement surgery) -know
the types of approaches available.
5) Pt had hx. of cholecystectomy - indications of
cholecystectomy - emergency vs elective
6) Complications of Cholecystectomy
7) Pt had hx.of hernia repair - indications
8) The first medication to administer when patient comes in
with chest pain.
9) What type of arrhythmia can cause patient to collapse.
