Selecting an assessment to use with a client can be an important part of a counselor’s work. A first step is to conduct a comprehensive assessment and identify the client's presenting problem. A common assessment used in this stage is an intake assessment. For this assignment, you will complete an intake assessment using one of the three case study presentations, identify a mental health issue, and recommend one formal assessment you might use to help diagnosis the client. To Prepare: • Complete the Required Reading for Week 3. • Locate the appropriate Mental Health Intake & Evaluation Form for your state or a state you’d like to see for an intake form. • Review the three case study presentations in 3.3 in the Neukrug text. Choose ONE that you will use as a case study for this activity. • Review Box 4.5, the Summary of Assessment Report in the Neukrug text. Assignment: Write a 1- to 2-page paper where you: • Summary of Assessment Report using the information from the case study you selected and the Mental Health Intake & Evaluation. By Day 7 of Week 3 Submit your Summary of Assessment Report Assignment.
