Tertiary Interventions
Consists of interventions aimed to facilitate the rehabilitation of the patients to the highest level of
functioning while addressing the risk factors that could further result in deterioration of the patients

ex. cardiac rehab programs, chronic disease management programs, pharmaceutical therapy
