NR 546 Master Problem List Client: PM Drug/Alcohol Use

Client: PM Drug/Alcohol Use: Client reported that he began using alcohol and marijuana at age 16. He began using powder cocaine nasally at age 17. At age 22, client began smoking crack cocaine. Client was also using cocaine intravenously in "speedballs". Last use was 3/5/23. Client reported sustaining an injury at age 18 which he was prescribed Oxycodone to treat. He reported that he began to abuse the prescribed medication and started using heroin at age 20. Last use was approx. 8 years ago when he was using 1 bundle of heroin intravenously per day. Client reported that he is prescribed Suboxone which has been assisting him in maintaining abstinence from opiates. Client was also prescribed Xanax at age 16 for anxiety and began abusing it at age 18. He reported that his last use was 3/5/23. He identified that Xanax was his drug of choice prior to his incarceration. Client reported having 5 overdoses over his lifetime and received Narcan each time
