Why: Asking this question is to assess for the potential of elderly abuse. This allows for an open ended discussion of how the patient is being treated. This method helps in recognizing signs of abuse and also builds trust between the patient and caregiver, simplifying the discussion of sensitive issues (Mydin, Yuen & Othman, 2021).

Why: Asking this question about the recent changes on the patient's mood helps to understand the emotional and psychological effects of her current living situation. It is important to understand her
emotional state, including signs of stress, depression, or anxiety, which can affect her overall health (Mydin, Yuen & Othman, 2021).

Potential Elder Abuse/Neglect (ICD-10 Code T76.11XA): Patient describing her daughter's harsh behavior towards her, financial abuse, and the unexplained bruises and abrasions.
Depression or Anxiety (ICD-10 F32.A): Implied by the patient's head down, avoiding eye contact, sad facial expression, and depressed mood.
What actions would you implement at this visit and why? Actions to Implement at This Visit:
Social Services Consultation: Connect with social services to evaluate the living conditions and investigate potential elder and financial abuse (Mydin, Yuen & Othman, 2021). Social service can also connect patient to food banks and meal delivery services to guarantee there is food availability.
Mental Health Evaluation: Referral to psych evaluation for the treatment of depression and anxiety (Mydin et al., 2022).
Dietary consult: Patient reported insufficient food availability. Dietary consult can assist with patient’s nutritional needs (Mydin et al., 2022).
