Theoretical Basis of community/ public
health nursing
what is the overall purpose of theoretical frameworks? - ANS-Theoretical frameworks
provide structure to our nursing practice and research - the backbone of EBP
what is the goal of the community health nurse? - ANS-•the goal of community health
nursing is to promote group health and to improve the health of the community
•Population health nurses go out into the community and meet them where they are. He
or She speaks to high school students about STDs or to the elderly about exercise and
health.
•They provided population focused care and find the needs of the community so that
they can serve them best.
definition of a theory? - ANS-•A theory is defined as a systematically interrelated
concept or hypotheses that seek to explain or predict phenomena
•For example: big bang theory to explain how our universe was born
•It is based either explicitly or philosophically on a conceptual model aka conceptual
framework
definition of a conceptual model/framework? - ANS-•A conceptual model/framework is
more practical! Provides direction for the nursing process and explains relationships
between person-nurse-environment
what are the three main parts of EBP? - ANS-research, nursing expertise, patient
values
•EBP helps bridge the knowledge (research) - practice GAP.
(Theories and conceptual models provide the framework for research and research is
one of the required three of EBP)
what did Florence Nightingale discover? - ANS-*Focuses on preventative care*
•Disease is more prevalent in poor environments and health could be promoted by
providing:
•Adequate ventilation
•Pure water
, •Quiet
•Warmth
•Light
•Cleanliness
420/1,000 soldiers died in her first two months - after interventions and within 6 months
= 22 deaths/1,000 soldiers
•As you hopefully know, Nightingale research began during the Crimean war were she
found that more soldiers died from infected wounds (even minor ones) then from War
itself.
•She realized that clean water, adequate ventilation, warmth, quiet, light and cleanliness
were keys to the soldiers survival.
•By realizing that poor environmental conditions = bad health she saved countless lives
•Today public health practitioners realize that where people live, work, and play impacts
their overall health
•Just like FN taught us over a century ago, to improve health we need to improve the
environment
•An example of Nightingales work in current day can be found in Cook County, Illinois.
What is this? - ANS-•Nurses used geographic information system technology to create
maps that indicated the level of walkability in low-income neighborhoods such as
availability of sidewalks and accessibility to both grocery stores and health services - by
understanding that environment and health are linked - the nurses advocated for
changes to those neighborhoods identified as having poor walkability. Nightingales
research influenced the research and provided the basis for research that improved
population health
describe Orem's self care model? - ANS-•Self Care was the focus of her concepts, that
of learned, goal-oriented actions to preserve and promote life, health, and well-being
•She theorized that those that we care for in the hospitals lack the ability of self-care.
She categorized people into 3 different systems:
•Nurse must provide total care as patient can provide no self-care
•Nurse and patient work together to perform self care
•And supportive-educative system where nurse works as a supporter and liaison to
patients self-care efforts.
•Persons can move through these through systems, for example if the self-care needs
become greater than the patients ability, nursing intervention is needed
