Agitated and Agressive Behaivor: pain and disorientation
Vision Loss: persons with poor vision can fall or trip, some cannot read lables
well and take the wrong medicine
Identifying the Person -✅✅--Patient will receive an ID bracelet which has the
persons name, DOB, room number, bed number, age, doctor, or any other
identifying info
-Use bracelet to identify before giving any patient care
-Compare identifying info on assignment sheet witht he ID bracelet
-Use at least 2 identifiers
Full name and DOB
CANNOT be bed # or room #
-Call the person by name when checking ID bracelet
Preventing Burns - ✅✅ -BURNS are the LEADING cause of death among
children and older persons
-Assist resident with hot foods and beverages
-Check the bath/shower water to ensure it is a safe and comfortable
temperature
-Supervise residents when they smoke
-Enforce restricted smoking areas for residents, visitors, and staff
-Monitor carefully any equipment which produces heat when in use
-Prevent any type of chemical burns
Preventing Poison - ✅✅ --NEVER leave potentially poisonous or harmful
substances at bedside
-Store harmful substances (cleaning supplies) in locked areas
-Remove wrappers and packaging from the trays of confused residents
-Monitor the placement of house plants
-Monitor meds in room
-Notify nurse STAT if resident ingests harmful substance
Preventing Choking - ✅✅ --Make sure the resident received the accurate and
appropriate diet
-Check with nurse before changing the diet or offering foods that are not in the
diet
-Make sure food is cut or chopped in small enough pieces
-Position the resident properly. NEVER feed a resident laying flat
-Alternate solid foods and liquid foods
-Feed the resident slowly , allowing them time to chew and swallow
, Preventing Falls -✅✅ --Shoes or slippers with non-skid soles
-Tie shoe laces
-Avoid long gowns and robes
-Avoid the use of throw rugs
-Wipe up all spills immediatley
-Encourage the use of hand rails
-Non-skid tips on canes, walkers and cair legs
