Care Study 1: Valerie B. aged 24 years.
Valerie B. was admitted to the community hospital, following surgery for her
appendix which burst before she could reach hospital. Valerie has mental health
problems which began when her parents died when she was six years old.
Valerie’s surgery was complicated due to her being obese. She has low self-
esteem and cannot see why she needs to lose weight or change her lifestyle.
Valerie lives in flat provided by a housing association but has been bullied by
local teenagers because of her obesity. Valerie’s surgical wound is healing, but
she does not want to go home because she is afraid that the bullying will start
again. She is unemployed and receives Universal benefit. Valerie has a hearing
impairment but has never been assessed for any support to improve her
hearing. Valerie is a vegetarian.
Case study 4: Billy G. aged 18 years.
Billy is homeless and was admitted to the community hospital by a passing
paramedic, when he had an asthma attack in the street. The city hospital did
not have beds and so a place was found in the community hospital. Billy left
home following an argument with his mother’s partner, which resulted in a
violent fight between Billy and the man. He has been living on the streets since
this incident. The weather is cold and damp which contributed to the asthma
attack. Billy has a visual impairment and finds street living difficult. He cannot
clearly read road signs or find his way around the city easily. Billy has
complained about stomach pains since being admitted to hospital, which could
be due to him living mainly on food from rubbish bins, thrown out by local
restaurants. Billy cannot go back home from hospital, because his mother has
changed the locks and has refused to visit him.
Explain why meeting the needs of the individuals
requires the involvement of different agencies.
Multidisciplinary teams are teams that work
together but have different roles and
responsibilities, for example, social workers,
psychologists, doctors, and youth workers are
examples of groups. These specialists have
specialized services that they offer to patients,
focusing on their areas of expertise to treat various problems that patients may face. Multi-
Agency have an effective way of supporting other needs and supports to improve life
outcomes for families and children. Multidisciplinary teams and multi-agencies help people
meet their needs. It is important for organizations to work together to help people live
healthier and to help them improve their quality of life. They can enable individuals to take
charge of their health by ensuring they have the resources and facilities they need to meet
their needs and by funding health services such as the NHS. The Care Act 2014 makes sure
service users are provided with appropriate care and support especially those with physical,
emotional, and mental health problems, and users who need accessible care.
, A variety of services and professionals must work together to meet the needs of service
users and provide quality care to service users with diverse and complex needs. Each agency
can be linked to health or social care. There are many organizations in the UK responsible for
the health and social care commission for example CCG is a group of GPs who work together
under the guidance of NHS England to provide the best care for the community as well as
they are in charge of the budgeting. According to HETT Insights (2024) their main job was
that they were responsible for any type of finance regard NHS England and for co-ordinating
care. This would be beneficial to Valerie and Billy because they don’t feel included in their
community, Valerie because she’d been bullied by people in her estate and Billy has been
kicked out by his mother which left him living on the streets with no support therefore the
CCG is responsible to make sure he’s getting
adequate care. By amending care for them, more
people with similar issues/situations are more likely
to be able to get the help they need. Another agency
would be the Local Authority as they work together
with our partner such as the NHS to support people
in their community, Valerie would benefit from this
because she’s not only on universal credits and can’t
afford to move independently but also her mental health is impacted but all the bullying she
suffers so it’s in her best interest to move into a new house which the local authority can
work in partnership with Valerie’s Social worker to find her a place to live where she doesn’t
dread coming home as well as the NHS to get her the help she needs to deal with her
hearing impairment. NHS England (2017) make it known that they strongly believe that both
need to work together especially with increased pressures that are being put on Health and
social care professionals. Lastly, another important element for multi agencies is Common
Assessment Framework (CAF) It’s used as a way to identify the types of needs the person
needs its mostly for those 0-19 years old as mentioned by Learning Ladders (2023) this is
because CAF allows easy and clear communication between multiple agencies. It also makes
it a lot less dauting for the patient because they only need to answer a few sets of question
once instead of being asked multiple times by multiple different services. CAF would benefit
Billy because as he’s 18 years old and it can help professionals be certain with the type of
help he would need and thorough with his evaluation and it’d show what professionals he
would need supporting him.

meeting the needs of specific individuals.
Multidisciplinary teams as stated by Macmillan Cancer Support (2023) refers to a group of
people from the health sector and the community who work together but have different
roles to support people with mild or severe illness, such as diagnosing a disease. Most
multidisciplinary teams in the health and social fields ensure that service users receive care
from professionals who know and understand each area to which they are assigned.
