1. What drugs are tocolytics? Terbutaline, mag sulfate, Procardia
2. Magnesium sulfate MOA: interferes with calcium uptake in myometrium
cells, reducing muscular ability to contract resulting in smooth muscle
relaxation (similar to calcium channel blockers)
3. Mag Sulfate dosage: Loading= to g IV over 30 minutes
Maintenance= g/hr. IV: 4-6, 2-3
4. Mag sulfate AE: Hot flushes, sweating, n/v, drowsiness, dizziness,
transient H TN, hypocalcemia, respiratory depression, hyporeflexia
5. Mag sulfate N': use infusion control, monitor maternal VS and FHR
6. Mag sulfate Toxicity Signs: respiratory distress, decreased LOC,
hyporeflexia, urine output > 30 ml, cardiac dysrhythmias
7. What to do if mag sult toxicity suspected? Discontinue mag sult,
administer calcium gluconate, notify provider, prevent respiratory or
cardiac arrest
8. What timeframe does mag sulfate provide neuroprotection to neonates?
after neonatal viability to 34 weeks gestation (reduces risk of cerebral
palsy)
9. Terbutaline (Bertine) MOA: beta adrenergic agonist, relaxes smooth
muscles
10. Initial dose and route of terbutaline: mg q : 0.25 mg SQ q
20-30 minutes x 2 hrs.
1 1. Maintenance dose and route of terbutaline: mg
Maintenance: 0.25 mg SQ q3-4 hrs.
12. What are the 3 ways terbutaline can be administered? SQ, IV, PO
13. Terbutaline AE: tachycardia, SOB, tachypnea, pulmonary edema,
palpitations
14. Terbutaline N': Monitor maternal VS and FHR
15. Procardia (Nifedipine) MOA: reduces contractions by inhibiting calcium
from entering smooth muscle cells
16. Nifedipine loading dose and route: mg : 30mg PO
17. Nifedipine maintenance dose and route: mg q : 10-20 mg
