Case Management

Case management refers to a process in medical care where a health specialist assists a

client develop a plan which fit in and organizes all supportive services essential in maximizing

healthcare as well as psychosocial results (Giardino & De Jesus, 2021). This process aids the

patient navigate a complex collection of services offered by their community (Giardino & De

Jesus, 2021). The following is an evaluation of how case management helps to care for patients

and a recommendation that the case management department should report to me- the chief

nursing executive of a healthcare delivery system- rather than the chief finance officer (CFO).

Case management is an integral part of providing healthcare, and it has become even

more critical in the current climate of tighter budgets and the need for healthcare organizations to

stay in business. Case management's primary goal is to ensure people get cost-friendly and high-

quality care (Hudon et al., 2022). Here, patients are connected with the right clinical services;

thus, the patient receives care in the best setting and speaks up for the patient's needs throughout

their care journey.

Case management gives a unique look at how a patient is cared for and how their health

is doing. This lets case managers find care gaps and coordinate resources so that patients get the

proper care in the most cost-effective way (Hudon et al., 2022). By finding gaps in care ahead of

time, case managers can lower costs related to services that aren't needed or extended hospital

stays and improve patient care.

Case management is also a cost-effective way to manage the care of patients with

complex or long-term illnesses. Case managers can determine the patient's needs, make a care

plan, and coordinate resources to ensure the patient gets the best care possible (Hudon et al.,
