Health assessment - ANSWER: A process involving gathering and analyzing health-
related information about a patient, which could be subjective or objective data.
Components of health assessment - ANSWER: Assessment (data collection),
Diagnosis (identifying the problem and developing a plan of care), Planning (setting
outcomes and goals, developing interventions), Implementation (performing
interventions), Evaluation (assessing effectiveness).
Subjective data - ANSWER: Information based on what the patient feels or
communicates.
Objective data - ANSWER: Information based on what is observed during an
examination.
Types of health assessment - ANSWER: Comprehensive assessment, Problem-
based/focused assessment, Episodic/follow-up assessment, Shift assessment,
Screening assessment/examination.
Three levels of health promotion - ANSWER: Primary Prevention (preventing disease
occurrence), Secondary prevention (early disease identification), Tertiary prevention
(minimizing disease severity and disability).
