EARLY DECELERATION: -slowing of FHR w/ start of contraction -due to compression of baby's head from contraction *no intervention necessary LATE DECELERATION: -slowing of FHR after contraction has started and prolonged return to baseline -due to uteroplacental insufficiency *place patient in side-lying position, administer IV fluids, d/c oxytocin, administer O2, palpate uterus for tachysystole, notify provider. VARIABLE DECELERATION: -transient, variable slowing FHR ("V", "W" on wave form) -due to umbilical cord compression -place patient in knee-chest position, trendelenburg, d/c oxytocin, administer O2 RUPTURE OF MEMBRANES: SROM spontaneous rupture of membrane: naturally occurring rupture of the fetal membranes during or after onset of labor, normal require NO intervention RUPTURE OF MEMBRANES: AROM AMNIOTOMY breaking the water of the amniotic sac intentionally during labor test amniotic fluid after for infection PLACENTA PREVIA: painless vaginal bleeding treatment: -tocolytics -IV fluids -blood transfusions C-SECTION REQUIRED PLACENTA ABRUPTION: 3rd trimester bleeding associated with severe abdominal pain GUSH OF BLOOD VBAC: vaginal birth after cesarean section candidates: 1-2 prior low transverse C-sections, no other problems LABOR PAIN MANAGEMENT: -epidural -butorphanol EPIDURAL: -eliminates sensation from umbilicus to thighs. Administer when mom is at least 4cm dilated -s/e: maternal hypotension, fetal bradycardia -nursing interventions: administer IV fluids, position mom on side, monitor maternal VS and FHR continually BUTORPHANOL: -narcotic opioid analgesic s/e for mom: Pain at the injection site Nausea and vomiting Drowsiness or sedation Dizziness Muscle weakness Itching Urinary retention s/e for baby: Slow breathing in the baby Sleepiness Poor feeding or breastfeeding Apnea POST PARTUM ASSESSMENT: BUBBLEHE -BREASTS: size, shape, firmness, redness, symmetry -UTERUS-firm or boggy, fundal height, midline or deviated (deviated sign of a full bladder) -BLADDER: void w/in 4-6 hours -BOWEL: bm w/in 2-3 days -LOCHIA: COCA, should be odorless, lightens over time, less than 1 pad per hour -EPISIOTOMY: REEDA - HOMAN'S SIGN: signs of DVT, lying supine, leg bent, dorsiflex, pain, look for edema, redness, warmth -RUBIN'S PHASES RhoGram: who qualifies Rh incompatibility (necessary for Rh negative mothers who give birth to an Rh positive baby; prevents hemolytic disease of the newborn) given at 28 weeks and w/in 72 hrs after delivery Postpartum pain control: ice first 24 hours, heat after first 24 hours, nsaids are #1 med for uterine pain POSTPARTUM HEMORRHAGE (PPH): S/S Excessive bleeding after childbirth; traditionally defined as a loss of 500 ml or more after a vaginal birth and 1000 ml after a cesarean birth POSTPARTUM HEMORRHAGE (PPH): NURSING INTERVENTIONS POSTPARTUM HEMORRHAGE (PPH): medications -oxytocin (Pitocin) -methylergonovine (Methergine) -misoprostol (Cytotec) IMMEDIATE NEWBORN ASSESSMENT: APGAR SCORING A= appearance (color all pink, pink and blue, blue [pale]) P= pulse (100, 100, absent) G= grimace (cough, grimace, no response) A= activity (flexed, flaccid, limp) R= respirations (strong cry, weak cry, absent) IMMEDIATE NEWBORN ASSESSMENT: skin findings normal purplish pink with acrocyanosis NEWBORN ASSESSMENT: NORMAL VITALS RR: 30-50 HR: 120-160 3 vessels of umbilical cord, we look for after the baby is born? 2 arteries and 1 vein NEWBORN MEDICATIONS: what and why Erythromycin eye ointment - the lower canthus of the eye so that the baby won't get gonorrhea or Chlamydia.
