Review of Systems is?

out or identify concurrent disorders
Able to capture physical or psychological s/s that were not identified in the HPI
sluggishly
What is Circumstantiality?
,Disturbance in the associative thought and speech process in which a pt
digresses into unnecessary details and inappropriate thoughts before
communicating the central idea.
Pt giving too much information and irrelevant to a question and then at the end

Phases of a diagnostic interview?
How long should they each last?
1. Opening phase-provider establishes rapport with pt, gathers background info
and sets agenda for interview. 10-15 min
2. Middle Phase-provider gathering information about the pt's current
symptoms, past psychiatric history, fam history, and relevant psychosocial
factors. 45 min - 1 hr.
3. Closing phase-summarization of key points, giving feedback to pt, next steps.
10 - 15 min
What are the tasks of the diagnostic interview?
1. establish a therapeutic alliance
2. Obtain the psychiatric database
3. interview for diagnosis
4. Negotiate a treatment plan with the pt
Difference between reflection and summarizing
, Reflection- provider repeats back pt's words, to reflect what they are hearing.
rewording or paraphrasing pt's words to let them know you have heard and
understood what was said
Summarizing- review portions of the conversation to assure provider has heard
and understood the pt. Pt can modify or clarify
What is different with regard to the MSE when performing it on a pediatric pt?
Includes parent-child interaction, separation and reunion and social relatedness
Difference between restricted and blunted affect?
Restricted - showing a limited range of feelings
Is less severe than blunted, they may show some feeling/emotions
Blunted affect - showing minimal intensity of feeling
Manifests as reduced affective responsivity, facial expression, body gestures
and voice patterns
Flat affect - showing no feeling
Difference between Insight and judgment
Insight - pt's understanding of how and why they are feeling the way they are
Judgment - refers to a pt's ability to make good decisions and act on them
Difference between flight of ideas and derailment
Flight of ideas - a succession of a number of ideas or associations that may be
hard to follow but there is a logical connection
