How does the health assessment start
• Nurses ASSESSING the client Can be full head-to-toe Usually more focused on

How should the exam room be set up
• The examination room should be warm and comfortable, quiet, private, and well lit.
How should the lighting in the exam room
Lighting with natural daylight is best, although it is often not available; artificial light from
two sources suffices and prevents shadows.
How should the examination table be set up
Position the examination table so both sides of the person are easily accessible
How high should the table be for examination
• The table should be at a height at which you can stand without stooping and should be
equipped to raisé the person's head up to 45 degrees.

How should you approach the patient
Answer all questions directly and honestly. Ask patient to put on their gown and empty
their bladder.

skin tone , rash , bruising and observing the patients mood )

(What I feel to assess texture temperature moisture , organ location, size, swelling and
anything abnormal)
