HEALTH HISTORY REVIEW for patients with chest pain
Dyspnea Past cardiac history Orthopnea High BP, cholesterol • Family cardiac history
Cough Fatigue Cyanosis or pallor Edema or swelling in legs or feet Recent weight gain
NECK VESSELS: INSPECTION for what
Inspect Jugular Vein & carotid arteries From JV you can assess CVP and judge heart's
efficiency as a pump
NECK VESSELS: INSPECTION for JVD
• JVD jugular vein distention • Position patient supine anywhere from 30-45- degree
angle • Remove the pillow to avoid flexing the neck • Turn the person's head slightly
away from the examined side
NECK VESSELS: PALPATION of what
carotid arteries
How should you Palpate carotid arteries
Palpate carotid arteries Gently palpate each artery medial to sternomastoid in neck •
Palpate only ONE carotid artery at a time to avoid compromising blood to brain
NECK VESSELS: AUSCULTATION of what artery
Auscultate carotid artery
Lightly apply bell of stethoscope over carotid artery at three levels:
Angle of jaw Midclavicular area • Base of neck
How should one use NECK VESSELS for AUSCULATION of carotid artery
atherosclerosis • Ask person to take a breath, exhale, and hold briefly while you listen
so that tracheal breath sounds do not mask or mimic carotid artery bruit
The heart extends from where to where
from the 2nd to the 5th intercostal space and from the right border of the sternum to the
left midclavicular line.
How to do a INSPECTION on ANTERIOR CHEST
Chest symmetry on inhale & exhale • Pulsations: you may or may not see apical
impulse, pulsation.created as left ventricle rotates against chest wall during systole
Auscultation of the heart
Identify auscultatory areas where will listen; these include four traditional valve "areas"
Erb's point is where and what sound is best heard there
3rd ICS can hear S2 the best 5th area
areas 2nd ICS right sternal border:
aortic valve area
2nd ICS left sternal border:
pulmonic valve area
4th ICS left sternal border:
tricuspid valve area
Fifth ICS at around left midclavicular line:
mitral valve area
