apparent cardiac or respiratory distress. Pt does not seem anxious or in pain
7. Pt appears comfortable...,next: Pt is well developed, well nourished, with appro- priate grooming
and hygiene
8. Pt appears well developed...next,: I'm going to ask you a few questions, to test your memory.
Where are we today, what year is it and what is your name. Pt is alert and attentive, oriented x3

expression, speech is normal, and no involuntary movements noted

11.How to start vital signs: Have you consumed any caffeine or nicotine in the past 30 mins
12.After asking ab nic/caffeine, next: I'm going to take your pulse, pulse (15 secs) x 4, pulse is
rhythmic and regular at 78 bpm
13.After pulse, next: I'm going to take your pulse another way, check rr for a full minute.
Respiratory rate is normal and rhythmic at 16 breaths per minute

15.How to start blood pressure,: State: if the blood pressure is abnormal I will take it in a
different position
