COMPLETE SOLUTIONS
Identify THREE assessment findings that would indicate the worsening fluid
status of Ms Cook
- changes in vital signs - we would imagine any loss of blood volume and therefore
preload will result in a decrease in BP and an increase in HR
- poor circulating volume may also result in pre-renal impairment - so reduced urine
output
- poor circulating volume can result in poor peripheral perfusion - ie reduced capillary
refill, decreased warmth and colour, decrease pulse pressure at extremities ie weak
pedal or radial pulses
- poor circulating volume will affect mean arterial pressures and therefore vital perfusion
to organs such as brain and heart - so assessment findings may reflect poor
oxygenation of these organs ie altered conscious state/confusion or ischaemic ECG
changes
Ms Cook is at risk of hypovolaemia. Provide TWO reasons as to why this is the
case
- Ongoing bleeding from the compound fracture of the left femur
- Potential for Internal bleeding due to trauma
- Significant blood loss at the scene- external bleeding
, If airway obstruction were present, identify TWO immediate nursing actions to
assist in ensuring a patent airway.
- Gentle suction being careful not to obstruct the airway
- Jaw thrust if required- NO head tilt /chin lift due to spinal precautions
What is the most likely cause for a potential airway obstruction in Mr Brown? (1
mark)
Foreign object obstructing the airway from facial trauma ie broken teeth
List TWO (2) priority nursing assessments you should undertake and a brief
rationale for each of them
a pain assessment using the PQRST
ECG
Explain TWO (2) priority nursing interventions that would assist Mr Stevens with
his symptoms.
- positioning patient in semi-fowlers position to maximise lung expansion and better gas
exchange to improve oxygenation
- applying oxygen to achieve sa02 >94% to decrease risk for cardiac ischaemia
- rest and reassurance to decrease cardiac workload
- administration of prescribed medications for ischaemic pain such as GTN
- call for help - escalate care to more senior nurse or doctor to review ECG
A burn where both the epidermis and inner dermis are destroyed is termed a
full-thickness burn
The first step in setting up an integrated Chest drainage Unit (CDU) is to:
fill the water-seal chamber to the level specified by the manufacturer
