Sub-Acute Rehabilitation (i.e. Kindred) - Answer: Patient is ready to leave the
hospital, but is not ready to be discharged directly home and may not be ready for
more intensive in-patient rehabilitation (described below). Patient may then go to
a Sub-Acute Rehabilitation facility
Criteria for admission and defining characteristics of a sub-acute rehab facility is

- Defined admission criteria based on diagnosis and there must be a plan for the
patient to be discharged to more independent living
- Must tolerate 3 hours of therapy per day, 5 days per week
- Therapy can be a combination of PT, OT, ST
- These rehab services fall under "Physical Medicine and Rehabilitation"
Acute Rehabilitation Hospital (Encompass Rehabilitation Hospital) - Answer: Based
