QUESTIONS AND ANSWERS GRADED A+
Anticoagulant, Antiplatelet, and Thrombolytic Drugs:
Question 1 of 5
Clopidogrel is an example of which class of antiplatelets?
Cyclooxygenase inhibitors
Anticoagulants
P2Y12 adenosine diphosphate (ADP) receptor blockers
Glycoprotein IIb/IIIa receptor blockers
P2Y12 adenosine diphosphate (ADP) receptor blockers
Clopidogrel is an example of a P2Y12 ADP receptor blocker.
Question 2 of 5
In which organ is clopidogrel activated for its drug effects?
Kidney
Stomach
Liver
Small intestines
Liver
Clopidogrel is activated in the liver primarily by the hepatic CYP2C19 enzyme system.
Question 3 of 5
Which phrases describe the main functions of antiplatelet drugs?
,Select all that apply.
Suppress the production of clotting factors
Decrease platelet aggregation
Prevent the conversion of fibrinogen to fibrin
Inhibit thrombus formation
Prevent antithrombin from converting to thrombin
Decrease platelet aggregation
Antiplatelets are drugs that decrease platelet aggregation. This prevents platelets from
sticking together to form a blood clot.
Inhibit thrombus formation
Antiplatelets are drugs that inhibit thrombus (blood clot) formation. When platelets
cannot stick together, a thrombus cannot form.
Question 4 of 5
Prophylactic therapy with antiplatelet drugs is indicated in patients with a history
of which conditions?
Select all that apply.
Peptic ulcers
Transient ischemic attacks (TIAs)
History of myocardial infarction (MI)
Intracranial bleed
Factor V Leiden deficiency
History of ischemic stroke
,Transient ischemic attacks (TIAs)
Patients with TIAs are at an increased risk for thromboembolic stroke. Antiplatelet drugs
are indicated for these patients to prevent future TIAs or ischemic stroke from a
thromboembolus
.
History of myocardial infarction (MI)
Patients with a history of MIs are at an increased risk for future MIs due to
thromboembolus. Antiplatelet drugs are indicated for these patients.
Factor V Leiden deficiency
Factor V Leiden deficiency is a hypercoagulable disorder which increases the risk for
blood clots. Antiplatelet drugs are indicated for patients with factor V Leiden deficiency.
History of ischemic stroke
Patients with a history of ischemic strokes are at an increased risk for a future ischemic
stroke. Antiplatelet drugs are indicated for these patients to prevent future
thromboembolism.
Question 5 of 5
After administration, when would the nurse expect clopidogrel to start having its
effect? __________ hours
Use numbers only.
The onset of action for clopidogrel is 1-2 hours.
, Side Effects and Adverse Effects of Clopidogrel:
-Clopidogrel is generally a well-tolerated drug and shares many of the same side
and adverse drug effects as aspirin. Common side effects include abdominal
pain, diarrhea, rash, and dyspepsia (indigestion).
-TTP is a rare (3 to 4 in 1 million) autoimmune blood disorder characterized by
clotting in small blood vessels of the body, resulting in low platelet count. Its
clinical manifestations include thrombocytopenia, neurologic symptoms, and
fever.
General Teaching:
-Inform patients that some people do not metabolize clopidogrel and will not
receive the same benefits. Inform them that testing is available to determine their
status.
-Teach patients that clopidogrel should be taken at the same time each day, with
or without food.
-Inform patients who are taking clopidogrel to manage ACS that they should also
take aspirin (75 to 325 mg) daily.
-Instruct patients to inform all health care providers if they taking clopidogrel,
especially before surgery is scheduled or before taking any new drug.
-Teach patients that clopidogrel should be stopped 5 days before any elective
surgical procedure
*Educate patients on the risks of bleeding and warn them
CASE STUDY
Mr. Dibecki, a 57-year-old man, has sustained a recent stroke for which he was
