, 1. How can I help you today?
I have had a cough for last three weeks which has been getting worse I have also
had shortness of breath when I'm walking to my next class at school I have to stop and
take a break.
2. Do you have any other symptoms or concerns we should discuss? no
but I did have a runny nose and congestion three weeks ago the condition
went away in a week but the cough is still there.

3. Is anything make your shortness of breath worse or better.
It gets a little better when I stop working and rest for a bit it’s a little worse at night.

4. Has there been any change in your shortness of breath overtime

The last few years i have had cough and shortness of breath but it got better after a few
days so I did not get checked. This time it’s lasting longer. I also did not want to stress
out my parents because they have to work so much so I never told them.

5. Anything make your cough better or worse?
Not really it has been constant but it has been getting worse over the past three
days and seems to be worse at night.
6. Do you have any pain or other symptoms associated with your cough?
Nothing but I am getting out of breath and I think that it may be related.
7. Are you coughing up any sputum? No
, 8. Treatments have you had for your cough? Nothing
9. Do you have any allergies? You mean like hay fever and stuff like that no
10. Are you taking any prescription medication’s? No still no prescriptions
11. Are you taking any over-the-counter or herbal medications. No
nothing like that
12. Are your immunizations up-to-date? I assume so
13. Now or have you ever smoked or chewed tobacco. No
14. Exposed to secondhand smoke? Yes my dad lost his job three years ago

so he has been doing our job since then. We had to move to an older
building, and it has cockroaches in it. Also the neighbors smoke inside the
building so I always smell smoke everywhere.

16. Did you first notice feeling of shortness of breath ? This has been
happening for three days now and it’s made me a little scared.

you have had? I have eczema but it is mild and I moisturize every day so it does not
bother me too much.
(FH) Tell me about the health of your grandparents, parents, and children.
Family history both my parents are healthy. My dad has eczema like me but he controls it
by moisturizing every day. I am not aware of my grandparent’s health.
