Involves communication - ask client questions about problem, listen attentively. Do not
make assumptions!
3. Find a Solution - think of as many as can. Decide which is most practical and helpful
and safe. Try solution to see if works.
4. Devise a Plan - use creativity to plan what you will do. Don't be afraid to try plan, if it
is safe. Record the outcome and ask/assess whether it worked.
Guidelines for Bathing Clients
1. Follow care plan for method and skin care products. Allow for choices whenever
possible.
2. Follow Standard Practices - ie. wear gloves when washing perineal area.
3. Make sure client fully understands procedure beforehand and talk to supervisor if
they do not.
4. Collect all needed items before starting. For safety, do not leave client.
5. Provide for privacy - close doors, shades, drapes, etc.
6. Cover client for warmth and privacy.
7. Wash from cleanest to dirtiest areas.
8. Use good body. mechanics.

6. Drainage or bleeding from wounds/openings
7. Swelling of feet and legs
8. Corns or calluses on feet
9. Skin temperature
10. Complaints of discomfort or pain
*If client urinates or defecates during bath:
Put on gloves, drain bath, cover client to keep warm, remove stool with toilet tissue,
dispose in toilet, clean and refill tub, help client back in, use clean washcloth.
Control verbal and nonverbal reactions to avoid embarrassment.
*Hardened secretions or stool:
Use unscented lotion or petroleum jelly to gently clean area.
*Client Erection:
Remain professional and calm, continue bath; or give some time/privacy if possible and
they would prefer -leave room/close door. Knock to re-enter.
SMART Goals
Help manage time and stress.
Goals should be:
1. Specific - they must be clear, give direction and focus. Ie. losing 5kg by end of March
2. Measurable - goals must have references which tell if you are making progress. Ie.
"5kg" and "by the end of March"
3. Achievable - challenging, yet achievable. Consider how much time and effort needed
and can be put in. May need 2+ parts. Ie. Investigate diet or exercise plan, determine
how much time will be needed to put into action.
4. Realistic - accounts for time, resources, and skills. Ie. losing 5kg in 3 months is
realistic, perhaps not if going on vacation for 1 month.
5. Timely - target dates for meeting goals increases commitment. Break goals into parts,
set schedules. As one part achieved, will gain confidence, be motivated to reach more.
Ie. break goal down into losing 1.6kg per month.
