NSG 201: Exam 1 Study Guide
What is a health assessment?
a systematic method of collecting and analyzing data. it is used to create a patient
centered plan of care
What are the components of a health assessment?
history (subjective data), physical examination (objective data), and documentation of
data
Comprehensive assessment
includes a detailed history and physical examination performed most commonly in a
primary care setting or on admission to a hospital/long term facility. This assessment
encompasses health problems experiences by the patient, health promotion, disease
prevention, and assessment for problems associated with known risk factors
Problem based/focused assessmet
involves a history and physical examination that is limited to a specific problem or
complaint. For example, a patient comes in with a sprained ankle. the assessment is
most commonly done at an urgent care/walk in clinic or an ED
Episodic/follow up assessment
this type of assessment is usually done when a patient is following up with a health care
provider for a previously identified problem. For example, a patient treated by a health
care provider for pneumonia might be asked to return for a follow up visit after
completing a prescription of antibiotics.
Shift assessment
When individuals are hospitalized, nurses conduct assessments each shift. The purpose
of the shift assessment is to identify changes to a patients condition compared to the
beginning of the patient's stay. The focus of this assessment is based on the condition
the patient is experiencing/being hospitalized for
Screening assessment/examination
, this is a short examination focused on disease detection. a screening examination may
be performed in a health care providers office. Examples include blood pressure,
glucose, cholesterol, and colorectal screenings
Define sign
clinical findings (objective) collected during physical examination
Define symptom
what the patient feels/communicates (subjective)
What are the different types of health assessments?
comprehensive health assessment, problem based or focused health assessment,
episodic assessment, shift assessment, and screening assessment
What are the three levels of health promotion?
primary, secondary, and tertiary
Primary health promotion
preventing disease from developing through promoting healthy lifestyle
ex: immunizations, pollution control, nutrition, exercise
Secondary health promotion
screening efforts to promote early detection of disease
ex: colorectal screening, mammography, blood pressure screening
tertiary health promotion
minimizing disability from acute or chronic illness or injury and allowing for most
productive life within limitations
ex: Diabetes mellitus management, cardiac rehab, hypertension management
What do nurses provide to help meet the health promotion needs of their patients?
education and care
How can data be organized?
by body system or conceptual
