Storing paper medical records requires an allotted space for
storage.
Determining the amount of space needed to store paper charts can
be difÏcult as the charts can vary in size, and you must take into
consideration the amount of space the facility has dedicated for
storing charts. Facilities with paper charts will ultimately utilize an
offsite storage company to send older charts for long term storage,
allowing for space in the facility for current and recently closed or
discharged charts.
Accessibility to a paper medical chart can be cumbersome. The
facility may keep active charts in a separate location from the
closed charts.
Accessing an active paper chart is challenging as many healthcare
workers may need the paper chart to document notes, complete a
chart review, and file reports. If any healthcare team member has an
active chart, the rest of the team members will have to wait for the
chart to be available. This can cause a delay in providing immediate
care, timely documentation, and increase the risk of a chart being
misplaced. Using an active chart checkout system will keep a location
on where that active chart is, when it is not in the designated location.
Ensuring the paper chart is not accessed by unauthorized personnel
is an extreme risk that will require due diligence from the whole
healthcare team.
