conditions (e.g., GERD, IBS, gallbladder disease, or past GI issues)
Medications: Current prescriptions or over-the-counter drugs
Family History: Any relevant hereditary conditions (e.g., GI disorders, cancers)
Social History: Lifestyle factors like smoking, alcohol use, diet, or recent travel
Review of Systems (ROS): Other symptoms that could be related (fever,
nausea, vomiting, weight loss, etc.)
, General Appearance:
Alert, well-nourished, no acute distress
Vital Signs:
Temperature: Normal or possibly elevated (if infection is present)
Heart Rate: Normal or tachycardic (if in pain or dehydrated)
