administered hormonal contraceptive to a patient under a protocol established by regulation of NV BOP
w/o prescription
-Complete 1 CE concerning self-administered hormonal contraceptive approved by ACPE or ACOG or
provided by school of pharmacy accredited by ACPE
-AND notify board his or her intent to dispense under protocol
-Maintain and keep proof of completion of course for at least 2 years
Once completed above:
-Complete a risk assessment questionnaire with patient who requests hormonal contraceptive
-Utilize a treatment algorithm to determine whether it is safe to dispense to patient
,-Provide certain records to patient pertaining to his/her request for self-administered hormonal
contraceptive
-Dispense if safe to do so
-May not dispense more than 12 month supply
-Provide information to patient concerning the use of contraceptive
-AND maintain certain records relating to dispensing contraceptive for at least 2 years
