PHARMACOLOGY
Subcategory • Short acting:-Nitroglycerine
• Long acting:-Isosorbide dinitrate, Erthrityl tetranirate, Pentaerythritol
tetranitrate, Amyl nitrate
MECHANISM OF • Also called nitites because the actual action is because of nitrites.
ACTION • Relax smooth muscles by releasing NO and syntheis of cGMP.
• Vasodilation reduce cardiac work load.
• Main effect is on coronary circulation.
• Dilation of intracranial vessels can cause headache.
ADME and Side • Oral and sublingual (preferred in acute attack).
effects • Inactivated by GIT and hepatic enzymes.
• Throbbing headache, dizziness, postural hypotension.
• Interacts with viagra to cause fall in b.p
ADDITIONAL USES • CHF, MI, Cynide poisoning
• In cyanide poisoning, nitrites convert Hb into methemoglobin.
Methemoglobin combines with the cyanide ions to form
cyanomethemoglobin which is relatively less toxic.
