HYSTERECTOMY
• Is surgical removal of the uterus
• It may be done either vaginally or abdominally
• The vaginal route is used when vaginal repair is to be done in addition to removal of
uterus
• Abdominal route is used in case of
- Large tumors
- Exploration of pelvic cavity
- Need to remove fallopian tubes and ovaries
INDICATIONS
Dysfunctional uterine bleeding
Endometriosis
Fibroids
Uterine rupture
Chronic PID
Malignancies in early stages like cervical cancer, endometrial cancer, ovarian cancer.
Uterine prolapse
TYPES OF HYSTERECTOMY
1. Sub-total hysterectomy – removal of uterus without the cervix rarely done.
2. Total hysterectomy (TAH/TVH) Removal of the uterus and the cervix
May be done vaginally or abdominally
3. Total abdominal hysterectomy with bilateral salpingo – oophonrectomy (TAH – BSO)
Removal of uterus, cervix, fallopian tubes and ovaries.
4. Radical abdominal hysterectomy
Removal of uterus, cervix fallopian tubes and ovaries ((TAH – BSO) plus removal of the
lymph nodes, upper 1/3 of the vagina (partial vaginectomy) and parametrium.
Commonly done in case of malignancies
5. Laparoscopically assisted vaginal hysterectomy
Performed with the assistance of a laparoscopy
Associated with rapid recovery short hospital stay and low incidence of post-operational
infection
