Discuss how the psychiatric mental health nurse practitioner could assist
the client in the creation of a more viable medication management plan.
For a client with schizophrenia who has minimal resources or income, the PMHNP
plays a critical role in creating a medication management plan that is both
accessible and effective. One way we could do that would be by finding a low cost
medication that is the most effective. However, considering that based on the
scenario that is being presented, we would consider that the patient does not
always have the means to get his medication as it is prescribed. At this point we
may consider long-acting injectable antipsychotics. They may at times cost higher
up front but it helps ultimately save on the cost of multiple prescriptions and
reduce hospital readmission rates due to improving adherence (Schneider-Thomas,
2022). Also collaborating with other pharmacies and such to see if they have any
discount programs available or patient assistance.
Write a prescription appropriate for the client with evidence support.
Risperidone 2mg PO QD for 30 days. I would do this first as my idea would be that
I make sure that the patient tolerates the medication and have some efficacy from
it. Eventually, in an ideal world I would see that they could tolerate up to 4mg and
see that it works for them so that I may consider placing him on Invega Sustenna,
the once a month LAI (Invega Sustenna, 2025).
Identify housing and support resources available in your community that are
relevant for this client.
Programs like Section 8 housing or public housing can help the client secure
affordable accommodations. There are also supportive housing programs that can
help with low income housing that has access to other social services. In the city of
Las Vegas there are some such as the Lighthouse Recovery Program that exist in
order to help this kind of population with getting back on their feet by providing
services that include social workers, case managers, etc (Las Vegas Rescue
Mission, 2025). Also the Southern Nevada National Alliance on Mental Illness
(NAMI) provides many resources on their website with housing resources, rent
resources, legal assistance, etc (NAMI, 2025)

The Clubhouse Model is a community-based rehabilitation model that provides
patients with mental illness a structured, supportive environment to help them live
and work in the community. This model could benefit this patient as it would assist
